DeBakey, as a leading vascular surgeon performing thousands of operations to repair blocked or damaged blood vessels, was the first to show that atherosclerotic disease was very often segmental, i.e., localized to one portion of the blood vessel rather than distributed uniformly. He first articulated this reality--and its implications for surgical therapy--in the early 1960s. In this 1978 article, he gave a more complete presentation on the topic, informed by an additional fifteen years of surgical experience and study.
