When Abraham Flexner, of the Carnegie Foundation, reported on his survey of the state of American medical schools in 1910, he noted that Johns Hopkins had created the best model for medical education. A follow-up report to the Rockefeller Foundation -- which was considering giving Johns Hopkins a $1,000,000 grant to further improve its medical program -- recommended that clinical professors be required to give up their private practices and work only for the medical school. Private practice, Flexner said, distracted the clinical faculty from doing the kind of important scientific research done by the non-clinical faculty. In this open letter to university president Ira Remsen, Osler argued that the full-time arrangement would be a disaster.
